CES Licensure Program Guides and Forms
Practicum
Everyone enrolled in a CES Licensure Program must complete a Practicum before they can obtain an Initial License.
Fitchburg State University
To pursue a Master’s Degree or a Certificate of Advanced Graduate Study, students must apply through Fitchburg State University as a non-licensure candidate through an Extended Campus Partnership Program.
Student Forms
- Student Request Form – This form is to be used by students for requests of exceptions to policy and academic regulation due to extenuating circumstances only.
- Leave of Absense Policy and Request Form – Students who must temporarily interrupt their studies because of a qualifying personal reason must complete a Request for Leave of Absence form indicating the length of time requested and date of return to active status.

MTEL
The Massachusetts Test for Educator Licensure (MTEL) is an important component of the licensure process. Successful completion of the Communication and Literacy MTEL is required for admission to all CES Licensure programs, and licensure-specific tests must be passed before cadidates can begin the Practicum. These tests are offered year-round at locations throughout Massachusetts and are designed to test candidates’ communication skills as well as their knowledge of subject matter in the licensure area. They consist of a combination of multiple choice and open-response items.
